“Adults integrated care in Camden: The opportunities and challenges” 

Camden Community Education Provider Network (CEPN) invites you to a Networking Event.

In the past year, local providers have been engaged to develop and implement a plan about how we can improve integrated working to care for adults. Work has focused on creating multi-disciplinary teams that will bring together GPs, social and home care workers, community nursing, therapists and palliative care services. This is based on the concept of health and social care professionals working with service users to ensure that their health and care needs are being met and that the right person is delivering the right care at the right time. 

At this event you will hear about the challenges and opportunities that have been faced by teams working together to improve care for people.

The event is open to all health and social care workforce in Camden, including those working inGeneral Practice, Care Homes, Voluntary and Community Sector, Secondary Care, Local Authority, Community Pharmacy, Dentistry, Optometry.
